One of the positive aspects of dining out with Bill is his ability to make server staffs wait on us properly or pay dearly. I've seen him order food that's NOT on the menu and get it.
Your server needed to find another line of work. Service obviously isn't her deal. If you should be so brave as to return to that restaurant you should ask for the floor manager right when they seat you. When he/she arrives, you tell them that last time you were here your service sucked SO much that you really are unhappy about returning. Tell him you really would like to be a customer, but if service isn't up to your satisfaction this time you'll be speaking to corporate. I can almost guarantee you service will be so good they'll be refilling your cups between sips.
There are only a couple of places we cant eat at anymore, but that's more because Bill made such a scene that we're truly afraid they'll spit in our food. The other places we enjoy excellent service and leave standard to generous tips.
I've never had real bad service at an applebee's. never had particularly good service either, but nothing as remarkably horrible as what you have described. I have never been particularly impressed with their food though so I seldom eat there.
I've had servers fired on the spot for bad service a couple times. I tend to get good service when I go out. I am infamous in my area for expressing my dissatisfaction when such is called for. I am also known as a generous tipper when that is called for.
Chain outfits tend to be less concerned however, with one man's complaints. And they tend to have very short memories. I try to avoid chain restaurants as much as possible.

Nytquill - I'm in Calgary. The wages hear are unbelievable. It's because there just arent enough people to fill the positions available. We're in the middle of a major growth spurt. Every eating place in town has a help wanted sign. Some of the fast food places are even offering scholarships to teen employees that stay with them more than one year.
I'm surprised that you say tipping is more generous in Canada but being in Quebec could easily make the difference. Here it is very rare to see any tipping at fast food places.
Fallyn.....You've just pointed out one of the advantages to small town living. Knowing all the neighbors makes a huge difference in many things.
